Nurses in Distress Everyday 

Nurses seems inevitable that caring for patients in the high-stakes context of COVID-19 will take its toll on the mental health of nurses. A public health emergency on this scale makes the threat personal for nurses, as well as one they manage professionally: with inadequate and insufficient personal protective equipment (PPE) reported in many areas, nurses are anxious about their own welfare and that of their families. 

Nurses make up the bulk of the healthcare workforce and are natural problem-solvers and innovators. We therefore stand out as indispensable at any time, but especially during a public health emergency. During the COVID-19 pandemic, nurses are experiencing pressure, fear, exhaustion, isolation and ongoing emotional trauma. This ongoing stress and trauma impacts your mental health, safety, and ability to provide the best possible care. Taking steps to manage your stress is just as important as taking care of your physical health! Many nurses are encountering unprecedented circumstances that may cause moral injury. This moral distress can be difficult to cope with and you may need additional support to address the damage it can cause. Symptoms of moral distress include self-criticism and intense feelings of shame, guilt, or disgust. It can also contribute to depression or post-traumatic stress disorder. 

The emergence of the COVID-19 and its consequences has led to fears, worries, and anxiety among individuals worldwide. The outbreak of COVID-19 has laid unprecedented psychological stress on health workers. Medical workers have been facing enormous pressure, including a high risk of infection and inadequate protection, overwork, frustration which is causing different mental health problems such as stress, anxiety, depressive symptoms, and fear. Very little is known about the perceived risk of COVID-19, fear and psychological distress among the healthcare workers during the emergency of the COVID-19 Pandemic. Measuring and understanding mental health aspect of health workers is essential to fight COVID-19 in long turn. Thus, the study aims to find out the status of perceived risk of COVID-19, fear and psychological distress, and associated factors among healthcare workers in Nepal during COVID-19 Pandemic. It is everyone’s bounded responsibility to ensure the high morale high of the doctors,nurses, and health workers to defeat the corona virus together.Health workers knows well how to take precautions.All safety measures are being taken by hospital staff on corona virus duty assure that they do not carry any infection. The discrimination would demoralize them and derail the health sector. Health care staff are on the front-line working day and night selflessly in this nonresidential crisis. They are the heart and the soul of the war against corona virus. During this dark episode, the strength, dedication and kindness shown by doctors, nurse and healthcare workers is humbling. In different part of the world, Doctors, nurses and other health workers are lionized and cheered by homebound individual for contributing front-line defense against Covid -19. 

Nurses often encounter similar situations in their careers. Despite their personal values, beliefs, and professional knowledge, they sometimes feel constrained and unable to do what they consider to be the "right thing" because of family preferences, workplace culture, lack of resources, institutional policies, and/or directives from supervisors. This phenomenon is known as moral distress. Nurses may feel alone when faced with this kind of dilemma, but the experience is believed to be part of daily professional life for some nurses. Moral distress varies between individuals depending on their own perceived obligations and values, and it has a negative impact on patient care and the nursing profession. Here, we'll examine moral distress and how nurses can address it 

How COVID-19 is affecting nurses’ mental health ? 
Worries and uncertainties about personal safety and the lack of PPE, workload, case complexity, skills mix, and loved ones’ health are among a vast range of stressors that cut across professional settings, Many nurses – mental health and community staff among them – have to deal with the stress of caring for patients and service users even while provision of PPE is inadequate.. 


Nine Healthy Ways to Deal with Distress:

1. Identify your needs. 
“When we are in distress, we need something,” said Radle, who practices with Eddins at Eddins Counseling Group.  She gave these examples: We may have an emotional need to feel accepted or heard. We may have a tangible need to have more help around the house. We may have an environmental need for peace and quiet. We may have a psychological need to treat ourselves with kindness. 
Naming your needs, Radle said, can be tough. In fact, most of her clients don’t know their needs. Instead, “They tend to get stuck on thoughts of, ‘I wish my life were different. I wish things weren’t this way. I wish I were more _____ or less ____. I just want to be happier.’” 
When you’re feeling distressed, Radle suggested asking yourself: “What do I need right now?” 
Your automatic response might be: “I need less stress in my life!” or “I just want to be happier!” 
If so, keep asking questions: “What does that mean exactly? What does that look like? What does that feel like? What does that entail? How might that be achieved?” 

2. Focus on what you want — not on what you don’t. 
When thinking about your needs, it can be more helpful to focus on what you need, instead of what you don’t need, Radle said. 
She gave this example: “Instead of saying, ‘I don’t want to feel lonely,’ come up with specific ways that you can feel more connected to, supported by, and engaged in your community, circle of friends, and/or family.” 

3. Honor your needs. 
After you discover what you need, honor it. When applicable, communicate those needs to others, Radle said. 
“If you don’t clearly communicate your needs, no one will know how to support you.” We can’t expect people to read our minds, she said. “That isn’t fair to them nor to ourselves.” 

4. Get moving. 
“When we’re highly stressed moving can help pump more blood and oxygen to the brain and shift into our senses and surroundings to feel grounded and safe,” Eddins said. 
What kind of movement you do depends on your preference and circumstance. For instance, if you’re feeling distressed at 3 a.m., it can help to stretch, walk around, jog in place or even wiggle your toes, she said. 

5. Develop a nurturing voice. 
According to Eddins, “Your inner nurturer starts by validating what you’re feeling [and] offers comforting and soothing statements [and hope].” 
She shared these examples: “You’re a good person going through a hard time. You’ll get through this. Let’s just take it one moment at a time; it will be OK.” 
You also might create a compassionate figure after a kind person you know, a spiritual guide or a fictional character, Eddins said. Turn to this figure when your thoughts are judgmental or self-critical, she said. 

6. Reverse the “Golden Rule.” 
Radle suggested reversing the Golden Rule, which states that we should treat others the way we’d like to be treated. “I find that most of my clients are far more compassionate towards others than they are towards themselves.” 
Radle defined kindness as being gentle and honest and honoring our needs. This may look different for every person. 
Kindness may include asking for help or saying yes or no, she said. For instance, you say yes to a massage and no to preparing a homemade dish for the office potluck. 
Kindness may include “telling yourself it’s OK that you’ve gained 10 pounds, that you’re still beautiful and still worthy of attention and affection.” 
It may include “acknowledging … that you did something that was hard for you to do, even if no one else noticed or even knew that it was a challenge for you.” 
It may include “forgiving yourself for making a mistake and for not being perfect.” 

7. Practice a soothing gesture. 
“Place your hand over your heart, imagine a positive memory you’ve had and just breathe in and out of your heart, feeling the connection between your hand and your heart,” Eddins said. 

8. Practice different perspectives. 
When we’re distressed, pain from the past may get reactivated, Eddins said. Then we may “create a number of stories around what is happening, which can be harmful to us and also inaccurate.” 
Instead, pause. Consider what you’d say to someone in the same situation, she said. “What would you say to a child? What other perspectives are possible? Can you think of three alternate neutral or positive explanations?” 

9. Ground yourself. 
“If your distress is so high that you’re feeling unsafe, and unable to access your other resources, you need to ground yourself first,” Eddins said. Grounding simply means anchoring yourself back to the present moment. 

Eddins shared these grounding techniques: 
  • Run cool or warm water over your hands.
  • Notice your body, such as practicing a body scan or clenching and releasing your fists.
  • Notice five things you can hear; five things you see in the room; five things you sense, such as certain textures touching your skin.
  • Remember words to an inspirational song, quote or poem that helps you feel better.
  • Remember a safe place and describe it in detail using your senses.
  • Count backwards in 7s or 9s.
  • Visualize yourself gliding away on skates, away from the pain you’re currently feeling.
  • Change the TV channel to a soothing show.
  • Change the radio station to something pleasant.
  • Imagine a wall as a buffer between you and your pain.

युद्ध लड्न हिँडेको सिपाई जस्तो भइयो,
परिवारबाट हेला पाउनेलाई पनि अगालियो ।
ज्यानको प्रवाह नगरी निरन्तर लागि परियो ,
सत्रु नै भएपनि माया ममता र उपचार गरियो ।।


 बदलामा

रोग फैलायौ हामीले भन्ने हल्ला सुनियो,
वरिपरिका आएर घर घेराउ र बन्द गरियो ।
कुटपिट गाली र धम्की पनि धेरै सहियो,
हिजाेआज त घरबाट निकाला भइयो ।।

तर        

खै यो समाज कहिले सम्म यस्तो हो ?
खै यो दूर्व्यवहार कहिले सम्म सहन पर्ने हो ??
आखिर कहिले ? आखिर कहिले ??

कोरोना भाइरस रोगको उपचार गर्नु भन्दा लाग्न नदिनु नै वेश । जुनसुकै रोग लाग्न नदिन मानिसले आफूलाई सक्षम बनाउनु पर्छ ।

के हो नोबेल कोरोना भाइरस ?
यो नयाँ प्रजातिको भाइरस हो जुन मानवजातिमा पहिचान भएको थिएन । कोरोना भाइरस श्वासप्रश्वासको माध्यमबाट सर्न सक्ने मानिएको रोग हो । यस भाइरसले सामान्य रुघाखोकी गराउन सक्छ । संक्रमित मानिसको श्वासप्रश्वास प्रणालीबाट बाहिर निस्किने थुक, खकार लगायतका छिटा (ड्रपलेट) बाट यो रोग निरोगी व्यक्तिमा सर्छ।

मानिस कसरि यो भाइरसबाट संक्रमित हुन सक्छ ?
मुख्य रूपले संक्रमित व्यक्ति खोकी, छींक वा बोल्दा उत्पन्न हुने थोपाहरू मार्फत फैलिन्छ। तपाईं दूषित सतहमा छुँदा र आफ्नो हात धुनु अघि आँखा, नाक वा मुख छोएर पनि संक्रमित हुन सक्नुहुन्छ। आफूलाई जोगाउनको लागि, कम्तिमा अरूबाट १-मिटर दूरी राख्नुहोस् र अक्सर छुने सतहहरूलाई कीटाणुरहित गर्नुहोस्। तपाइँको हात सफा गर्नुहोस् COVID-19 भाइरसले संक्रमित धेरै व्यक्तिहरूले हल्का देखि मध्यम श्वास रोगको अनुभव गर्नेछन् । पाको उमेरका व्यक्तिहरू, र हृदय रोग, मधुमेह, दीर्घकालीन श्वास रोग, र क्यान्सर जस्ता अन्तर्निहित मेडिकल समस्याहरूको साथ गम्भीर बिरामी हुने बढी सम्भावना हुन्छन्।

कोरोना भाइरसको लक्षणहरु के हो ?
  •  ज्वरोआउनु एक सयदशमलब ४डिग्रीफरेनाइट भन्दा बढी ज्वरो आउनु· खोकी लागने
  • टाउकोदुख्नने
  • सुख्खा खोकी
  • थकान
  • दुखाइ र पीडा
  • घाँटी दुखाइ
  • पखाला लागने
  • स्वाद वा गन्ध को घाटा
गम्भीर लक्षण:
  • सास फेर्न कठिनाई
  • छाती दुखाई
यदि तपाईंसँग गम्भीर लक्षणहरू छन् भने तत्काल चिकित्सकीय सहायता लिनुहोस्। सधैं तपाईंको डाक्टर वा स्वास्थ्य सुविधा भ्रमण गर्नु अघि कल गर्नुहोस्।

  • जेष्ठनागरिक, दीर्घरोगी
  • मुटुरोग
  • सुत्केरी र गर्भवतीआमाहरु, बालबालिका
  • दीर्घकालीन किडनी रोग
  • दीर्घकालीन अवरोधक फुफ्फुसीय रोग (सीओपीडी)
  • अंग प्रतिरोपणको कारण कमजोर प्रतिरक्षा प्रणाली
  • मोटोपना
  • हृदयको गम्भीर अवस्थाहरू जस्तै मुटुको विफलता वा कोरोनरी धमनी रोग
  • सिकल सेल रोग
  • टाइप २ मधुमेह
  • सिस्टिक फाइब्रोसिस
  • उच्च रक्तचाप
  • एचआईभी
  • डिमेंशिया
  • कलेजो रोग
  • गर्भावस्था
  • क्षतिग्रस्त वा दागको फोक्सोको टिश्यू (पल्मोनरी फाइब्रोसिस)
कोरोना भाइरसबाट कसरि आफु र अरुलाई बचाउने ?
  • नियमित रुपमा साबुन पानीले राम्रोसंग हात धुने ।
  • आफ्नो हात सफा राखौँ।हातले नाक–मुख छुन रोकौँ। संक्रमित व्यक्तिको नजिक नजाउँ, संक्रमित व्यक्तिको थुक वा खकारका छिटाआँखा, मुख र नाक सम्म नपुगोस्भनेर सतर्क रहौं। विश्वस्वास्थ्यसंगठन, नेपाल स्वास्थ्य तथा जनसंख्यामन्त्रालयले निर्धारण गरेको मापदण्डअनुसार मास्कलगाउने, हात सफा गर्नेर सामाजिक दूरी कायम गर्ने अभियान पालना गरौं ।
  • फ्लु जस्तो लक्षणहरु देकेयमा चिकित्सकसँग परामर्स लिने ।
  • मास्क लगाउने, बारम्बार साबुनपानीले हात धुने वा स्यानिटाइजरको प्रयोग गर्ने, भीडभाडमा नजाने, शंकास्पद बिरामीकोसम्पर्कमा नबस्ने, घर बाहिर निस्कन परे न्यूनतम दुई मिटर सामाजिक दूरी कायम राख्दै आफ्नो काम गर्ने। यति हुँदा हुँदै पनि कोरोनासंक्रमण भइहाल्यो भने भरखरै सरकारले जारी गरेको प्रोटोकलमा रहेर अस्पताल वा उपयुक्त स्थान भए सम्बन्धित स्वास्थ्यकर्मीको सल्लाह होम आइसोलेसन बसने ।
  • नेपालमा प्रयोग गरिने मसला र प्राकृतिक जडीबुटीको प्रयोगबाट जीवनाशक गुणप्राप्तहुन्छ। रोग प्रतिरोधक शक्ति वृद्धि गर्न सघाउँछ । तर अहिलेको कोरोना भाइरसको संक्रमणबाट जोगाएको हालसम्म कुनै प्रमाण फेला परेका छैन। अहिलेको कोरोना भाइरसको संक्रमणबाट जोगाएको हालसम्म कुनै प्रमाण फेला परेका छैन।
  • COVID भाइरस मुख्यत: नाकबाट डिस्चार्जको माध्यमबाट फैलिन्छ जब संक्रमित व्यक्ति खोकी वा छींक लाग्दछ, त्यसैले यो महत्त्वपूर्ण छ कि तपाईंले श्वासप्रश्वासको शिष्टाचार पनि अभ्यास गर्नु पर्छ (उदाहरणका लागि, खोकी द्वारा)
  • यस समयमा, COVID-19 को लागी कुनै खास खोप वा उपचारहरू छैनन्। यद्यपि, सम्भावित उपचार गर्ने धेरै चलिरहेका क्लिनिकल परीक्षणहरू छन्। WHO ले क्लिनिकल खोजहरू उपलब्ध हुने बित्तिकै अद्यावधिक जानकारी प्रदान गर्न जारी राख्नेछ।
  • कम्तिमा १ मिटर (feet फिट) दूरी आफु र अरु बीच राख्नुहोस्। जब कसैलाई खोकी लाग्ने, छींक लाग्ने, वा बोल्ने तिनीहरूले भाइरस समावेश गरेको नाक वा मुखबाट सानो तरल ड्रपलेटहरू स्प्रे गर्दछन्
  • भीड ठाँउमा जानबाट जोगिनुहोस्। जहाँ मानिसहरू भीडमा भेला हुन्छन्, तपाईं COVID भएको कसैसँग घनिष्ठ सम्पर्कमा आउन सक्ने सम्भावना धेरै हुन्छ र १ मिटर को शारीरिक दूरी कायम गर्न गाह्रो हुन्छ।
  • हातले धेरै सतहहरु लाई छुन्छ र भाइरस लिन सक्छ। एक पटक दूषित भएपछि, हातहरूले भाइरसलाई तपाईंको आँखा, नाक वा मुखमा सार्न सक्छ। त्यहाँ बाट, भाइरस तपाइँको शरीर मा प्रवेश र तपाइँलाई संक्रमित गर्न सक्दछ।
  • तपाईको स्वास्थ्य लाभ नभएसम्म खोक्ने, टाउको दुख्ने, हल्का ज्वरो जस्ता सामान्य लक्षणहरू भए पनि घर र स्व-पृथक रहनुहोस्। कसैलाई तपाइँको आपूर्ति ल्याउन छ। यदि तपाईंले आफ्नो घर छोड्नु आवश्यक छ भने, अरूलाई संक्रमित हुनबाट जोगिन मास्क लगाउनुहोस्। अरूसँग सम्पर्क बेवास्ता गर्दा तिनीहरूलाई सम्भावित COVID-19 र अन्य भाइरसहरूबाट बचाउनेछ।
  • तपाईंलाई ज्वरो, खोकी र सास फेर्न गाह्रो छ भने, चिकित्सा उपचार खोज्नुहोस्, तर सम्भव भएमा टेलिफोनबाट कल गर्नुहोस् र स्थानीय स्वास्थ्य अधिकारको निर्देशनहरू पालना गर्नुहोस्। तपाईंको क्षेत्रको अवस्थाको बारेमा राष्ट्रिय र स्थानीय अधिकारीहरूसँग सबैभन्दा पछिल्लो सूचना हुनेछ। अग्रिम कल गर्दा तपाईंको स्वास्थ्य सेवा प्रदायकलाई चाँडो तपाईंलाई सहि स्वास्थ्य सेवामा डाइरेक्ट गर्न अनुमति दिनेछ। यसले तपाइँको रक्षा गर्दछ र भाइरस र अन्य संक्रमणको प्रसार रोक्न मद्दत गर्दछ। 
  • विश्वस्त स्रोतहरूबाट नवीनतम जानकारी राख्नुहोस् । जस्तै डब्ल्यूएचओ वा तपाईंको स्थानीय र राष्ट्रिय स्वास्थ्य अधिकारीहरू। स्थानीय र राष्ट्रिय अधिकारीहरु लाई राम्रो सल्लाह मा राख्न को लागी तपाईको क्षेत्र मा मान्छेहरु को आफ्नो सुरक्षा को लागी गर्नु पर्छ|
सेनिटाइजरको सुरक्षित प्रयोग
  • रक्सीमा आधारित हात सेनिटाइजरहरू बच्चाहरूको पहुँचबाट टाढा राख्नुहोस्। सैनिटाइजर कसरी लागू गर्ने र यसको प्रयोगको अनुगमन कसरी गर्ने भनेर उनीहरूलाई सिकाउनुहोस्।
  • तपाईको आँखा, मुख र नाकलाई छुने बित्तिकै एल्कोहलमा आधारित हात सेनिटाइजर प्रयोग नगर्नुहोस्, किनकि यसले जलन निम्त्याउन सक्छ।
  • COVID-19 लाई बचाउन सिफारिस गरिएको हात सेनिटाइटरहरू रक्सीमा आधारित हुन् र त्यसैले ज्वलनशील हुन सक्छ। आगो चलाउनु वा खाना पकाउनु अघि प्रयोग नगर्नुहोस्।
अनुहारको मास्कले तपाईंलाई संक्रमणबाट बचाउँछ ?
  • तपाईं भाइरस फैलाउन सक्नुहुन्छ जब तपाईं कुरा गर्नुहुन्छ वा खोक्नुहुन्छ । तपाईंलाई यो थाहा छैन कि तपाईंले संक्रमणका लक्षणहरू देखाउँदै हुनुहुन्न भने।

Nurse is a Person who provided care to individual or community in the problematic situation as well as healthy conditions. In the old Days, the nurse we known as women who took care of the sick injured, and dying person without any formal Knowledge. Nursing is being a Major profession in providing health Care of Preventive, Promotive, Curative and Rehabilitative services. The Scope of nursing is very broad and highly demanded.

Nursing encompasses so much more than just medical care- it has to do with the human touch as well. When a patient needs a drink of water or a painkiller, it is the nurse whom they turn to. Nurses look after even the most intimate needs of patients, and they do so with compassion and thoughtfulness. Tasks that range from coaxing the patient to eat, telling a joke while drawing blood, to helping the patient to take a few steps after a surgery, all fall within the nurses’ responsibilities.

While it is the doctor who manages the overall healthcare plan, nurses are the ones who carry it out; and in doing so they spend the most number of contact hours with the patient. Ninety-nine per cent of patient interactions in hospital are with their nurses, and patients learn to trust their swift handling of any problems they may be facing.

Nursing is a health care profession that involves years of training and continued specialized education to care for patients in a variety of settings. Nurses are often the first healthcare professional that patients meet. They are a technical expert, an educator, a counselor and a resource for the family, using all senses to better care for a patient.

Nursing profession is a career in which one has the opportunity to give care to others in need, share health information with others to help them live better lives, and grow professionally in almost any way one can dream possible. Whether one is interested in research, bedside nursing, or advanced practice, nursing provides the opportunities to do all of these things. Another nice thing about nursing is that nurses are able to be many different kinds of nurses throughout their career. For example, one could be a pediatric nurse for a few years, then decide to be a critical care nurse for the next few years, and then decide to become a nursing instructor! It is an amazingly, rewarding career. One piece of advice to always remember: Don't forget the basics. Everyone wants a caregiver who truly cares about their well-being and comfort.

Nursing focus is on the whole patient, thereby setting itself apart from other disciplines through the positive caring approach. A profession is the ability to acquire knowledge in areas of science and the nursing profession is one that is grounded in theories, sciences, math, biology, and anatomy and physiology. Nursing is a discipline that focuses on alleviating pain and suffering through protection, promoting health, wellness, and prevention of illness and injury. The nursing profession is an advocacy in the care of individuals, families, communities, and populations. The profession advocates education, prevention, collaboration, coordination of care, and adheres to a high standard of care. Today, the nursing profession transformation is a vision of Florence Nightingale’s showing in the hospital designs, evidence-based medical care, and holistic patient. Nurse work to promote health, prevent disease and help patients cope with illness. They are always advocate and health educators for patients, family, and communities. Nurse develop and manage nursing Care plans instruct patients and their families in proper care, and help individuals and groups take steps to improve or maintain their health they spend much of their time on care. Nurse motivates the people to find their health needs, increase interest to adopt healthy life style and increase awareness regarding health ,sanitation, hygiene etc. Until the people are interested and become aware of their needs, they will not consume the health service provided to them.

Nurses always reflects the trust that the public hold in nurses; and truly, whether by the bedside or in the executive boardroom, nurses are the most trusted in the healthcare system in any country. Patients look to nurses for care during their most vulnerable times, and the nursing profession has proven time and again that they do, indeed, deserve this trust!

Nurses must necessarily draw on this single minded trust that patients place in them, by advocating them on health issues and doing all they can to improve the welfare of the community at large. Recovering patients are more likely to listen to the words of advice from the nurse who has been at their bedside during their stay in hospital. They will follow the patient care advocated by them, as they perceive that their nurses honestly care for them and want their well-being.

However tired they may themselves be, nurses are trained to be cheerful and pleasant and to extend compassionate care at all times. When you are not in the best of health, a smiling face can make all the difference to your quick recovery! At times when the patient’s health prognosis is not so positive, it is again only nurses who can be relied upon to tell the truth, clearly and correctly. Nurses often help patients to make informed decisions about their continued healthcare. For this reason, patients learn to expect honesty from their nurses.

Do anything wrong, they can be trusted to do all the right things they are only Nurse. To stay on top of the profession and keep themselves abreast of all the advancements in the field, nurses must undertake professional development courses. This will help them to keep up the trust that the community has placed in them and to carry out their duties to the best of their ability.
